Being too lean is also considered as a disorder.
 After lots of efforts and  treatement not gaining the weight  can be a symptom of any chronic disease. Anorexia or faulty eating habits, or too much desire for junk food , vigorous dieting, fasting, hard working exercises, heredity may be causes  of muscle wasting.
In Ayurveda classics it is named as Karshya/ karshyata if excess emanciation occurs called as Atikarshyata.

In 21 st chapter of sutrastan of Charak samhita, among 8 undesirable diseases or conditions explained one of those is Atikarshyata.The etiological factors for Atikarshyata may be divided into two groups- excessive expenditure of calories and very less quantity  intake of calories.
Either of these conditions lead to Mal nourishmen of body.
Weakness,lean and thin structure of body, weak musculature and feable bone,less internal vigour, strength of body is poor, weak sustaining capacity,weak immunity prone to many diseases. According to Ayurveda Vayu plays  an important role in  pathogenesis of karshyata.
Less formation of Rasa dhatu due to diseases or due to Mal nourishment.
Further categarised krishata is a Rasa pradoshaj vyadhi( Charak sutra.28/10: Sushrut sutra.15/37,24/8)
Emaciation is outcome of loss of fatty contents  or loss of adipose tissue components of subcutaneous  fat, which laying beneath the outer covering of the body. Extreme weight loss,leanness or thinness.
Generally the wasting of body is caused by hampered nutritional requirements at the tissue level and excessive starvtion.
Wasting or leanness is an improper nourishment which is commonly seen in various clinical conditions such as poverty, a veriety of gastrointestinal disorders, various eating disorders, prolonged fever,malignant diseases, endocrine disorders, chronic infections,autoimmune disorders,as well as parasitic infections. The malnourished person prone to a lot of cardiovascular, integumentary & urogenital diseases, disturbances in blood circulation, problems related to serum electrolyte,serum protein are commonly observed in emaciated person.
Such type of individullals are commonly suffer from infections due to deranged immune power and swelling.

Management:
Too lean patients are brought to good health through re-nourishment or re-introducing nourishing liquids & food to the body & check the process of catabolism.
It starts with laghu aahar i.e choose the food stuffs which are light to digest and can assimilate easily  even after its high  intake. 
Gradually increase the calories,By starting from a glass of water  followed by mixed with lemon,salt,and finally reach to juices,vegetable soups,pulses soups, mixture of pulces and rice by frying with ghee and pepper.Later on administer cow milk,soups of meat ,pulses soups with butter and salt.( Means food which can digest easily should take first following by the heavy to digest one).
If status of emaciated is improved,try to introduce small amount of solid meals  at the frequent interval and try to avoid a large  amount of solid food at a time.( Fod timing should be planned).
Dietary regimen should be focused on proteins ,fat , carbohydrates,multi vitamin and many for the management of the emaciated person .In this way gradual step by step increase of food pattern will be benificial to the emaciated one.
Because the weak digestive fire, deficient energy,may  causes slow process of digestive and total metabolism is slower. So excessive fatty  food items & excess fibers dominated grains & vegetables are to be avoided.

Patients of emancipation also recommended lots of sleep,rest,relaxation, councelling.
Patient should be free from worries exertion,for that pranayam,yoga can give significant improvement.

Oil masssage to body,Rasayanas(Rejuvnating medicines ) may be helpful.
Aswagandha( withenia somnifera, Shatavari ( Aspragus racemosus),Bala( sida cordiholia)etc like many herbs are recommonded in powder form with milk.

Brimhan basti that is enema of nourishing herbs also can  be give significant results.
 Santarpan ( nourishment by all means is the key to flourish the emaciated body. 
Many ayurvedic protein powders,immuno boosting products , Chyavanprash,are helpful in karshyata.
