In 21st chapter of sutrastan of Charak samhita under the group of chapters named 'Rogchatushka' i.e. the tetrad on guidelines on management of diseases. After enlisting eight undesirable physical appearance based on criteria like body height,body mass. Complexion& presence of hair .The extreme presence or absence of these parameters leads to undesirable physical changes.
Among these sthaulya i.e. morbid obesity ,and extreme emanciation occurs the highly existing disorders in this modern , fast era,where everyone is busy with there race of success, great negligence towards food , sleep and other activities can be seen,result of that many lifestyle disordrs are appears which are tedious to manage,leads to morbidity.
We can conclude that Charak was the first who studies genetic, hereditary ,and endocrinal disorders first in an anscient era,even in absence of skilled tools,equipments or technology.
So we can say that the recent concept of " metabolic syndrome" was already recognised in Ayurved.
Among all others, Atisthool / morbid obesity have the most prevalence , associated with several life threatening complications like Diebities, Hypertension,coronary artery. diseases,joint disorders,skin diseases, anorectal problems, hormonal imbalanced many times leads to Infertility also and many more.
Now a days complete world is focused on these life style metabolism related disorders,it's preventative measures, complications like paradiabeties, Diabeties ,congestive heart failure , cardiomyopathy ,arrythmia,sudden death.
Obesity is emerged as a serious health issues in both developed and developing nations
According to recent studies done by WHO about 500 millions adults ( approximately 1 in 10 adults) are obese,higher rate in women than men.
In past two dacades obesity rate in children and adolescents is doubled.
In old age patients obesity occur in age group of 50-60 in high income countries.
In Ayurveda classics Sushrut has considered Rasa dhatu as the main culprit for both obesity and emaciation (
Rasa nimmitameva sthaulyam karshyam cha)
In obesity obstruction of channels by excess accumulated medas ( fat) ,which.deranges the movement of vata is spescially confined to koshtha( abdominal viscera) , resulting in stimulation & hastening of digestive process through abnormality, increases agni( pitta) ,leads to excess hunger & thirst and the persons eats more food to gain weight. An excessive increase in adipose tissue and vitiation of tridosha causes severe diseases in obese people.
Extreme obesed has 8 inherant defects which are thoroughly described in Ayurved classics Charaka , symptoms or difficulties due to overweight or obesity. Those are
1)
Aayurhaas i.e. life span( longavity) of individual get diminished.
2)
Javoparodh (
utsah haani or hampered mobility)- loss of energy or loss of enthusiasm for any work.
3)
Krichchavyavay - (sexual impairment)
4)
Daurbalya. I.e. Debility ( Too much weakness or loss of muscular strength)
5)
Daurgandhya (bad odour from body)
6)Sweda (sweating profused or perspiration)
7)kshudha (excess hunger)
8)Pipasa(excess thirst)
For all these disorders too much fat disposition is the main reason .
Also disproportionately increased fat occurs mainly arround buttocks, abdomen & breast which becomes pendulous and the person suffers from improper metabolism and energy.
Reasons behind obesity-
Over-Nourishment due to intake of heavy ,sweet,cold & fatty diet,lack of physical exercise etc. In Ayurveda literature explained as
Atipoornahaar over nourishment or over feeding )
Gurusnigdhamadhoor Aahar- consumption of too much heavy to digest, Fatty,sweet ingredients in diet.
Avyayam-Lac of exercise.
Divaswapna- To take day sleep.
Nitya harsh- Fearless life
Chintahin- Worry less life.
Beejswabhav- Heredity
Symptoms of obesity as per Sushrut classics-
Kshudrashwas( short breathing),
pipasa( thirst), kshudha( hunger),
Nidra( over sleeping), sweda ( sweating),
Daurgandhya ( foul odour from body),
krathan( snoring),gatrasaad( feels loss of energy),
gadgadatva ( difficulty while talking),
kriyamandata( slower activities),
vyavayhaani( sexual impairment),
alpapraan( short life span).
They are very prone to Dibetes,fever,fistula, Abscess and various Vatavyadhi. Due to blocking of channels all disorders are difficult to cure.

Treatement:
Guru aahar diet which is heavy to digest,or diet containing raphez,more fibrous diet which couldn't accumulated for long-time,and excrete easily along drag out wastes in the body.
Also diet should be consists of low calories.
Also Basti i.e enema of herbal decoction made up of drugs posses properties like dry, scrapping agents.
Powder massages also helps to liquify the fat accumulated below skin.
Treatement of obesity mainly consist of diet& physical exercise for short term short
Short term dietary control is very difficult to an obese person & it requires exercise under the guidance of trend instructor; Physical exercise plays a key role to reverse obesity,as exercise involved brain along with digestive system which counteract metabolism along with weight reduction and balances nutrition.
There is a hypothesis that presumed that physical activity contribute both visceral adiposity and cerebellar brain changes because in the area of cortex and hippocampal dentate gyrus of brain show enhanced hypothesis & neurogenesis in response to physical exercise training.
Medicine- Guduchi,Bhadramusta,Takrarishta,madhujal( water with honey), vidanga,shunthi,kshaar,madhu,yava( barely or oats), aamalak,bilvadipanchmool,agnimantha, Shilajit, guggulu,Gomutra,triphala,rasanjan,lauhabhasma,kulathya,patol,medomaansakaphahar aahar,arishta prayog.
Madhoor(sweets),sheeta( cold),snigdha( oily),guru( heavy), picchhil( slimy,sticky), foodstuffs causes accumulation of fat,as thease properties of food good hevy to digest and which takes more time for absorption of its nutrients, obviously ut het accumulated or stored for long time which are converted into fat.
Ruksh udvartana are advocated obese patients as an external purificatory measures, while Vaman,VIRECHANA and Asthapana Basti as internal, p
possesses good stamina and strength,they should be treated with Vaman and Virechan karma.
Non unctous,warm &strong enema are advocated in obese.Yogic practices have a significant impact on the physical ,mental, emotional and spiritual health of the individual. It is reported that a significant improvement in the levels of blood pressure,LDL, cholesterol & BMI can be noted after three months of residence therapy consisting of vegetarian diet,kriya yoga.
